FASNY Accepting Nominations for Fire Service Community Achievement Award

(NEW YORK) - Each year, the Firemen’s Association of the State of New York (FASNY) presents the Fire Service Community Achievement Award to an individual who is an outstanding member of his or her community and volunteer fire department. A successful candidate must possess many years of experience, serving his or her community and volunteer FD by taking initiative and leading by example. This individual must volunteer with various community-based, religious and/or charitable organizations.
In addition, the nominee must be active in his or her local volunteer fire department and involved in all aspects of volunteer fire service. He or she may serve as a line officer, a social officer, or in different positions on numerous committees. As an active, long-time member of the volunteer fire department, the nominee shows total dedication to the volunteer fire service.
If you know anyone who fits these criteria, FASNY would like to learn more about your nominee. Applications must be postmarked by March 1, 2010.
The nomination packet for the nominated individual should include the following:
Nominating Sponsor’s Information:
- Name and affiliation (rank if applicable)
- Contact information
Nominee’s Information:
- Name, address, phone number and fire department
- Current employment
- Fire service background including offices and committee assignments
- Number of years served in department, date joined and current position
- Outstanding accomplishments in the department and community
- Other community service: any volunteer organizations served, titles and dates
- Personal history including military service, education, employment and other related
information
- Endorsement letters including those of support from the fire service, community service
organizations, community leaders and others
- Additional references and supporting material: newspaper articles, photos, videos, etc.
When submitting a nomination packet, please check that it has been:
- Signed by either the president, chief, or secretary of the submitting organization
- Printed on the official stationery of the submitting organization with all necessary contact
information attached
- Mailed to FASNY c/o Fire Service Community Achievement Award

A volunteer fire company or fire department, or a volunteer county association or sectional association may only submit one candidate annually. The nominating sponsor will be allowed to update the nomination packet for the candidate.
